Elmwood Village offers 1, 2, and 3-bedroom Garden Homes designed for privacy and independence. These totally handicapped accessible, quality built homes are absolutely stunning and offer all of the comforts of home including walk-in closets, cozy fireplaces, spacious kitchens and dining areas, indirect lighting, and so much more.

Assisted Living
A lifestyle alternative for seniors who are finding it too difficult to live at home. The combination of housing, personalized support services and health care are designed to assist with daily living activities such as eating, bathing, dressing, laundry, housekeeping, and assistance with medication administration. Now seniors have the opportunity to live as independently as possible in a safe and secure home-like residential setting.
Note: Some Elmwood Assisted Living communities offer skilled nursing care on a limited basis, along with Alzheimer's and Dementia/Memory Care.

Elmwood offers nursing home services on both their east and west campus in Green Springs. Each campus serves a certain population and is designed to help care for those who may require special nursing or rehabilitative care, and other supervised needs. Personal care services include: assistance in walking, bathing, dressing, and eating, with special diets available as prescribed by a physician. With the emphasis on quality living, the aim is to help a person care for himself or herself to the best of their ability. However, most residents usually come for long-term care resulting in permanent residency.
Our east campus Nursing Home specializes in caring for frail seniors, ages 75 and older, along with those suffering from Alzheimer's and other forms of dementia.
The west campus Long-Term Care facility is located within our Healthcare Center and provides care for residents' ages 18 to 75. This unit specializes in caring for those with spinal cord injuries, are comatose or cognitively unaware, or need special respiratory care. Twenty-four hour nursing care is provided in both facilities.
Care can be paid by the individual, and may be funded by Medicaid or Medicare programs, or through private long-term care insurance. Elmwood is Medicare and Medicaid certified.

Day & Respite Care (Short Term Care)
Day Care provided to those residents who may live at home but need occasional assistance when personal caregivers or family members are unavailable. Also available for those with *Alzheimer's or memory impairments. (*Tiffin and Fremont only)
Program offers:
- Flexible hours-minimum of 4 per day available 7 days a week

Alzheimers & Memory Care
Specialized care for memory-impaired individuals provided in a safe and secure home-like environment. Elmwood offers a separate neighborhood uniquely designed to meet the special needs of these residents. Each Reminiscence Neighborhood is designed to be both safe and stimulating. This distinctive environment provides cues and reminders to the residents, allowing them to be involved in the activities of daily living with the level of safety and security they need.
Each Reminiscence Neighborhood provides the following services and amenities:- Ongoing, individualized assistance with personal care

- Incontinence management
- Administration of medications and treatments
- Delicious, nutritious meals and snacks

Skilled Nursing & Rehabilitation Services
Elmwood offers Skilled Nursing and Rehabilitation Services on both their east and west campus. Specialized care for those who require physician ordered skilled nursing care typically ordered after a 3-day hospital stay. An average stay is 120 days or less. Physical, occupational, and speech therapy services are available onsite, six days a week. This unit specializes in post -operative orthopedic surgery care. Other nursing care services available include: IV/TPN Therapy, dressing changes, special diets, wound care, complex respiratory services, Hospice, & respite care. Warm-water aquatic therapy is also available onsite. Twenty-four hour nursing care is provided in both facilities. After an individual receives their skilled nursing services, they are often discharged to home, an assisted living or a long-term care nursing home, all of which are available in Elmwood Communities.
Care can be paid by the individual, and may be funded by Medicaid or Medicare programs, or through private long-term care insurance. Elmwood is Medicare and Medicaid certified.

Specialty Hospital - LTACH (Long-Term Acute Care Hospital)
Elmwood's Specialty Hospital provides specialized care services for patients requiring extended medical care and rehabilitation. Those with catastrophic or acute illnesses and injuries can work toward recovery. We serve patients who typically have multiple injuries and/or illnesses that require acute nursing care. Inpatient Dialysis and Wound Care Specialists are available. The average length of stay is 25 days. The Specialty Hospital is Medicare/Medicaid approved and has been providing healthcare and rehabilitation services since 1983, when it was designated one of Ohio's first Long-Term Acute Care Hospitals.

Outpatient Therapy & Specialty Services
Our state-of-the-art rehabilitation facility includes three (3) large therapy clinics and an aquatic therapy pool. Physical, Occupational, Aquatic, Hand, and Speech Therapy are provided. All therapy services require a physician's order.
Physical / Occupational Therapy: Rehabilitative services that use specially designed exercises and equipment to help patients regain or improve their physical abilities.
Back Therapy: A full-time, certified McKenzie Diplomat Back Specialist, one of only 315 worldwide is dedicated to treating your injury and offer preventive education.
Hand Therapy: A Certified Hand Therapist will help relieve pain, restore movement, and hand functions allowing you to regain confidence to perform daily tasks.
Speech Therapy: Assisting individuals of all ages, our specialists will help to restore speech after a stroke or injury. Assist individuals with autism, speech impediments, and other speech disorders.
Aquatic Therapy: Elmwood's Wetlands Aquatic Therapy Center offers a 30' x 40', warm-water therapy pool for great muscle strengthening and pain relief. See full description under Specialty Services.

Specialty Services
Aquatic Therapy Center: Elmwood's Wetlands Aquatic Therapy Center features one of the largest, state-of-the-art therapy pools in the area offering a 40 foot by 30 foot warm-water pool with a 30 foot by 15 foot movable floor. This hydraulic-powered floor enables access for all participants, regardless of their ability to enter the water. It can be raised to deck level for entry and lowered to any depth from zero (0) to seven (7) feet deep. Water temperature remains at a constant 92 degrees.
A section of the pool has a water massage area with built-in benches and adjustable, resistive water jets. The constant pool depth in the main pool area and in the water massage area is 3 feet, 6 inches.
Open swim available for therapeutic purposes. Fee applies. Accessible locker rooms and changing areas are also available.
Participation in Elmwood’s Wetlands Aquatic Program requires a physician’s referral for occupational or physical therapy and a physician’s approval for aquatic exercising. Treatments are structured to meet the individual’s goals and capabilities and are coordinated with other therapy treatments. Offered through Outpatient Services.
Sleep Center: Elmwood's Feathers Sleep Center can assist those who may suffer from Sleep Apnea or other sleep disorders. Signs of sleep disorders include morning headaches, feeling drowsy upon awakening, and experiencing memory difficulties or poor concentration throughout your day. To be tested, you must first contact your family physician since a prescription is required. Our private bedrooms offer a quality, queen size bed with pillow-topped mattress, flat screen TV, and other amenities. Elmwood's Director of Pulmonary Services will oversee all studies. Test results received quickly with consultation to follow. Accepted by most insurances. Offered through Outpatient Services.
Hearing/Audiology Services: Hearing and Audiology services are available in our Outpatient Services unit. Diagnostic hearing evaluations, hearing aid prescriptions, fittings, adjustments, repairs and wax removal service available. Day Springs Program: Innovative Habilitation program that helps Developmentally Disabled adults learn a variety of day skills while enhancing each individual's abilities in a fun and positive way. Programs include: Fitness program, Crafts, Horticultural program and more. Day Springs Hours: M-F, 9 am - 2 pm. Both East and West Campus facilities are utilized for this program.
